Clinical pilot

An 8‑week clinical pilot in your clinic

An 8‑week clinical pilot in which a participating outpatient clinic uses Cognitai in routine practice. It evaluates feasibility, acceptability and early impact on continuity of care, and tailors the platform to the clinic’s workflows and clinical approach.

Our founding clinical pilot round is underway.

Background and rationale

A lot of information, rarely brought together.

Psychotherapy is a longitudinal process: change unfolds across sessions and in the time between them, and continuity of care is a core dimension of its quality.

Over the course of care, clinically relevant information accumulates from many sources:

  • Intake and client history
  • Session content and clinical notes
  • Self-reports and standardized assessments
  • Worksheets and between-session activities
  • With the client’s consent, biometric data from wearables and health trackers

This information is clinically meaningful. Clinician feedback on between-session work, for example, has been associated with lower anxiety and depression scores in the following week (Espel-Huynh et al., 2024, Journal of Medical Internet Research).

Yet it is rarely integrated. Mental health clinicians report that patient-generated data is already part of their workflow, but its integration and use are not optimized, and they call for better support with interpretation (Wu et al., 2020, JMIR Formative Research). Synthesizing the full longitudinal record, within and between sessions, is difficult within the time constraints of routine practice. Without support, continuity can come to depend on what the clinician recalls and on the most recent note, which limits how continuous and evidence-informed care can be.

The platform

Cognitai brings the record of care together.

Cognitai is a clinical intelligence platform that supports evidence-informed, longitudinal care. It brings the record of care together and makes it interpretable for the clinician.

  • Before the session

A preparation brief on what has changed across sessions and between them, and what may deserve attention.

  • During the session

Assistance in capturing the session’s context, the clinician’s observations and key moments of insight (Aha Moment!), with transcription started by the clinician with the client’s consent.

  • After the session

Assistance in analyzing the session and connecting its evidence, for informed, clinically useful documentation aligned with the requirements of the clinician’s professional regulatory body, reviewed and approved by the clinician.

  • Between sessions

Check-ins and activities aligned with the client’s needs, recent sessions and the clinician’s approach.

  • Traceable

Every insight is traceable to its source.

  • Non-diagnostic

    Cognitai does not diagnose.

  • Clinician-controlled

Clinical decisions remain with the clinician, and nothing is filed without their approval.

It can be tailored to the clinic’s workflows and therapeutic approaches.

Traceable by design

You can see where every insight comes from.

In PrepKit, every finding rests on evidence from more than one source, and each piece can be opened. Point at a finding, or tap it, to see what it rests on.

Aims and design

Four aims, and a design built to learn.

The clinical pilot has four aims.

  1. 01To assess and optimize the feasibility and acceptability of Cognitai in routine practice.
  2. 02To explore its contribution to continuity of care and to clinicians’ longitudinal understanding of their clients.
  3. 03To explore change in the outcome domains selected with the clinic.
  4. 04To tailor Cognitai to the clinic’s different clinical workflows, priorities and therapeutic approaches.

Design

A single-site, uncontrolled pre–post evaluation embedded in routine care, using mixed methods. The clinic is compared only with its own baseline. Aims and measures are agreed with the clinic before the pilot begins. As a pilot, it is not designed to establish efficacy.

  • Single site
  • Uncontrolled pre–post
  • Compared only with your own baseline
  • Aims and measures agreed with you first
  • Not designed to establish efficacy

Setting and participants

An outpatient mental health clinic, with at least three participating clinicians, taking part voluntarily and with informed consent.

Methods and analysis

Quantitative
Validated, standardized instruments selected with the clinic, at baseline and close, plus clinician-reported time.
Qualitative
Semi-structured interviews with clinicians, using thematic analysis.
Analysis
Descriptive analysis of change from baseline, without causal inference.

For your clients

Let your clients feel understood.

  • They walk in, and their clinician already knows their week.
  • Continuity of care is a core dimension of the quality of psychotherapy.
  • This clinical pilot can measure continuity of care, alliance and engagement between sessions.

For your clinicians

Let your clinicians feel empowered.

  • Every session begins prepared, each insight traced to its source.
  • Clients already share the data; the bottleneck is using it.
  • This clinical pilot can measure workload, burnout and the usefulness of insights.

The 8 weeks

Eight weeks, from scoping to a written summary.

  1. Before start

    Scoping

Agree aims, outcome domains and measures.

  2. Week 0

    Baseline and onboarding

Workflow mapping, baseline measures, educational material.

  3. Weeks 1–8

    Implementation

Use in routine practice, with support. Mid-point review at week 4.

  4. Week 8

    Close

    End-of-pilot measures. Semi-structured interviews.

  5. After close

    Reporting

Written summary of findings for the clinic.

Outcome domains

Seven outcome domains. Two to four, chosen with you.

Selected with your clinic, typically two to four of the following.

  1. 01Continuity of careAs perceived by clinicians and clients
  2. 02Therapeutic allianceFrom clinician and client perspectives
  3. 03Premature termination of therapy
  4. 04Clinician burnout and workload
  5. 05Perceived usefulnessFor session preparation and treatment planning
  6. 06Workflow efficiencyTime spent preparing, documenting and following up
  7. 07Client engagement between sessions

Ethics, data and governance

Only what you choose to share, reported in aggregate.

  • Information you choose to provide

The evaluation uses only information that the clinic and its clinicians choose to provide to the Cognitai team, with consent. Client-reported measures, where selected, are provided to the Cognitai team only in anonymous, aggregate form, solely for the purpose of the evaluation.

  • Aggregate, never ranked

Results are reported to the clinic in aggregate, never per clinician and never ranked.

  • Encrypted, stored in Montréal

Data is encrypted and stored in Montréal, Québec. Cognitai is built to meet Québec’s Law 25 and PIPEDA requirements.

  • A quality-improvement evaluation

The pilot is a quality-improvement evaluation. Its results are not published. Any future publication would present aggregate findings only, would name the clinic only with its written permission, and would require prior review by an independent research ethics board.
